This initiative brought together hospital and primary care partners to develop national, evidence‑based Medicines Stewardship Toolkits to support safer transitions of care.
Designed for implementation through hospital Medicines and Therapeutics Committees, the resources provide practical guidance and education to improve medicines management across key therapeutic areas.
