CATAG has developed this practice tool to share resources with Medicines and Therapeutics Advisory Committees and hospitals to assist to identify people at “hidden risk” of cumulative medicines toxicity. This will support implementation of local medicines stewardship programs and promote safe and appropriate deprescribing.
